औद्योगिक परिसर के लिए अनुज्ञप्ति की मंजूरी या नवीनीकरण हेतु आवेदन-पत्र

1 औद्योगिक परिसर का पूरा नाम / Name of Industrial Premises
2 पते संबंधी विवरण / Address Particulars
(एक) औद्योगिक परिसर का डाक का पूरा पता तथा स्थिति / Postal Address & Location of Industrial Premises
(दो) औद्योगिक परिसर का वास्तविक स्थिति का पता (यदि भिन्न हो) / Actual Location Address (if different)
(तीन) आवेदक का पूरा पता / Applicant's Full Address
3 वित्तीय वर्ष के दौरान किसी भी एक दिन नियोजित किए जाने वाले कर्मचारियों की अधिकतम संख्या / Maximum number of workers employed on any one day during the financial year
4 उस व्यक्ति का पूरा नाम तथा निवास स्थान का पता जो अधिनियम के प्रयोजनों के लिए नियोजक होगा / Full name & residential address of the person who shall be deemed Employer for the purposes of the Act

6 क्या नियोजक के वित्तीय संसाधन (उदाहरणार्थ चल तथा अचल सम्पत्तियों के ब्यौरे तथा मूल्य, बैंक निर्देश, आय कर निर्धारण आदि) / What are the Employer's financial resources (e.g. details & value of movable and immovable properties, bank reference, income tax assessment etc.)
7 क्या नियोजक का व्यापार तथा पण्य चिन्ह अधिनियम, 1958 के अधीन पंजीयित व्यापार चिन्ह है / Is the Employer's trade a registered trademark under the Trade and Merchandise Marks Act, 1958?
8 पूर्ववर्ती वित्तीय वर्ष के दौरान औद्योगिक परिसर में निर्मित बीड़ियों या सिगारों या दोनों का मूल्य / Value of bidis or cigars or both manufactured in the industrial premises during the preceding financial year
9 उद्योग में आवेदक का पिछला अनुभव / Applicant's previous experience in the industry
10 क्या ओद्योगिक परिसर का प्रस्तावित स्थल किसी विद्यमान औद्योगिक परिसर के स्थल में परिवर्तन करके बनाया जाना है / Is the proposed site of the industrial premises to be created by altering the site of an existing industrial premises
11 क्या आवेदक द्वारा आवेदन पत्र की तारीख से ठीक पूर्ववर्ती बारह मास की अवधि के दौरान कोई औद्योगिक परिसर बन्द किया गया था / Was any industrial premises closed by the applicant during the twelve months immediately preceding the date of the application
12 तम्बाकू प्राप्त करने का स्रोत / Source of procuring tobacco
13 क्या आवेदक द्वारा निर्मित बीड़ियों या सिगार या दोनों की स्वयं आवेदक द्वारा या मालिक अथवा व्यापार तथा पण्य चिन्ह अधिनियम, 1958 के अधीन पंजीयित व्यापार चिन्ह में पंजीयित उपयोगकर्ता अथवा किसी अन्य व्यक्ति द्वारा बिक्री तथा हाट व्यवस्था की जाएगी / Whether the bidis/cigars manufactured by the applicant will be sold and marketed by the applicant, the owner, a registered user under a trademark registered under the Trade and Merchandise Marks Act 1958, or any other person
